The principal can witness passport forms by appointment only. Please call the school office on 041 9804578 to make an appointment if you need a passport form signed for your son/daughter.

Please read the notes on the passport form carefully, as the principal cannot sign the form if all the requirements are not met. Passport documents must be signed in the presence of the principal. Photo ID is essential.
